If you are a victim of sex trafficking and/or severe forms of trafficking in persons, you may request TEAM block adverse information that resulted from trafficking from your consumer report.

To assist us in processing your request, please provide the following information in your communication:

1. A copy of one of the following identification documents as Proof of Identity:
   1. Driver's License
   2. Government-issued ID
   3. Passport
   4. Social Security card
   5. Birth certificate
2. A copy of your Victim Determination Documentation
3. A list of the report information that resulted from trafficking and should be blocked, including information from after exploitation
